Radan Medicine Professional Corporation, operating as Radan Medical, and Dr. Aleksandar Radan are committed to protecting the privacy, confidentiality and security of the personal information and personal health information entrusted to us.
This Privacy Policy explains how information submitted through radanmedical.ca, huroncircumcision.ca, or otherwise provided to Radan Medicine Professional Corporation is collected, used, disclosed, retained and protected.
Our information practices are governed by Ontario’s Personal Health Information Protection Act, 2004 (“PHIPA”) and other applicable privacy laws.

Information Collected Through the Website
Depending on how you use the website or communicate with us, we may collect:
The parent’s or guardian’s name;
The baby’s name;
The baby’s date of birth or expected date of birth;
Telephone number;

Preferred method of communication;

Information included in an inquiry or booking request; and
Other information reasonably necessary to respond to the inquiry, assess eligibility, arrange an appointment or provide medical care.
Because the information is being collected in connection with a medical service, some of the information submitted may constitute personal health information.
Please do not submit detailed medical information through the general website form unless the form specifically requests it. The website form is not intended for emergencies or urgent medical concerns.
How Website Form Information Is Received and Stored
Information submitted through the website form is sent to an email account used by Radan Medicine Professional Corporation.
The submitted information may be contained in the email message and may also be temporarily processed or retained by Squarespace as part of operating and delivering the website form.
Email and online forms are not completely secure. There is a risk that information transmitted electronically could be intercepted, misdirected, delayed or accessed without authorization.
Radan Medicine Professional Corporation takes reasonable steps to protect information received by email, including restricting access to authorized individuals and using password-protected accounts and devices.

Communicate with other health-care providers when authorized or permitted by law;

We do not sell or rent personal information or personal health information.
Consent
By voluntarily submitting information through the website, email, telephone or another method, you consent to the collection, use and disclosure of that information for the purposes described in this policy and explained at the time of collection.
Where information relates to a baby or child, consent must be provided by a parent, guardian or another person legally authorized to act on the child’s behalf.

To professional advisers, insurers, regulators or legal representatives where reasonably necessary;

With the consent of the patient’s parent, guardian or authorized substitute decision-maker.
Service providers are only provided with information reasonably necessary to perform their services and are expected to protect that information appropriately.
Squarespace and Other Technology Providers
The website is hosted using Squarespace. Squarespace may process website form submissions and related technical information in order to operate and deliver the website and its forms.
Email, website and technology service providers may process or store information outside Ontario or outside Canada. Information processed in another jurisdiction may be subject to the laws of that jurisdiction.
No internet-based system can be guaranteed to be completely secure. For sensitive or urgent matters, please contact the practice by telephone rather than using the general website form.
Email and Electronic Communications
By providing an email address or telephone number, you authorize Radan Medicine Professional Corporation to use that contact information for communications relating to your inquiry, appointment, payment or medical care, unless you advise us otherwise.
Email and ordinary text messaging may not be secure. Messages may be intercepted, delayed, misdirected or accessed by someone other than the intended recipient.
Please notify the practice promptly if your contact information changes or if you believe a message has been sent to the wrong person.
The website form and email account are not continuously monitored.

Safeguards
Radan Medicine Professional Corporation uses reasonable administrative, technical and physical safeguards appropriate to the sensitivity of the information in its custody or control. These may include:

Retention and Disposal
Personal health information that becomes part of a patient’s medical record will be retained for the period required by applicable legal and professional obligations.
Website inquiries that do not become part of a medical record will be retained only for as long as reasonably necessary to respond to the inquiry, arrange services and meet legal, accounting, security or administrative requirements.
When information is no longer required, it will be securely deleted, destroyed or anonymized, subject to applicable retention requirements and technical limitations relating to email backups and other system backups.
Accessing or Correcting Personal Health Information
A patient, or a person legally authorized to act on the patient’s behalf, may request access to or correction of the patient’s personal health information.
Requests should be submitted to the Privacy Contact. Radan Medicine Professional Corporation may require proof of identity and proof of authority to act for the patient.
Access or correction may be limited or refused in circumstances permitted by law. A reasonable fee may be charged where permitted.
Privacy Incidents
If personal health information is lost, stolen, used without authorization or disclosed without authorization, Radan Medicine Professional Corporation will investigate and take appropriate steps.
Affected individuals and applicable regulatory or professional bodies will be notified where required.
